NHS PATIENT DATA
NHS England is introducing a modern data service whereby certain information about patients will be automatically extracted from medical records. The identifiers to be extracted are NHS number, date of birth, postcode and gender which will allow patients’ GP data to be linked to their hospital data. No free text in patients’ records will be extracted, only coded information about referrals, NHS prescriptions and other clinical data.

Patients have the right to prevent their confidential information from being shared or used for any purpose other than providing care, except in special circumstances. If you do not want information that identifies you to be shared outside your GP practice, please let the surgery know and we will make a note of this on your records.
Patients will also be able to restrict the use of information held by other places they receive care, such as hospitals and community services. Please let the surgery know if you wish to restrict the use of this information. Your choice will not affect the care you receive.
